Medical history template: Tracking my dog’s health

Keeping close tabs on Scout’s medical history became more important than ever when she was diagnosed with idiopathic epilepsy (meaning she’d need additional veterinary care throughout her life) and then again when we moved into our van full time (meaning we might have to visit unfamiliar clinics while on the road).
